[MERGE] nested-progress-bar api

This adds the nested progress bar api and converts the existing code to
use nested_progress_bar everywhere that a progress bar was being
created, and to release them.

Test suite passes, nuff' said I hope.

It does not change choices about whether to pass-in/create-new or stack:
so tree transform will behave as it did before(for instance).

The core patch is the api change, the update patch is the change of
callers to the new api.
