[MERGE] Handle SMTP connection errors nicely, especially if default used

Hi all,

This patch updates SMTPConnection so that it produces nice errors if it is
unable to connect to the default or specified SMTP server.  If it is unable to
connect to default server, the message suggests setting smtp_server.

This should address the SMTP connection errors that Alexander encountered.  It
still attempts SMTP connections to localhost on Windows, but I think this
handles the general case best.  Other OSes may lack local SMTP servers too, and
we should try to handle that situation gracefully.
