In Breezy's Evolution, I cannot create a remote calendar.  When I select new -->
calendar, then for "type" I switch from "on this computer" to "on the web", the
updated window doesn't have a field for the remote URL of the new calendar
(which is the behaviour in Hoary's evolution).
