This is a serious problem when using older (low resolution) displays. Often the initial presentation of a fixed-size window will have the control buttons both above and below the screen, so that there is no way to either re-size or exit the window.
 For example try System - Preferences - Sound on Dapper with 800 x 600 resolution.  Many of us older folks prefer to run at low resolutions to help our bleary eyes.  The system should adapt.
