I can confirm this bug. 
Just to clarify: the problem is that totem-gstreamer is laggy and gstreamer-xine is not.
The audio is in sync but he video is dropping frames. 
gsteamer 0.10.5-0ubuntu1
totem-gstreamer 1.4.0-0ubuntu5
totem-xine 1.4.0-0ubuntu5
amd64 nvidia
All upgrades in dapper as of today.
