Could you provide your relevant gconf settings (/apps/evolution/*)?
Maybe I have inadvertently configured something that blew away my
notifications or am missing a plugin or something.

While browsing through my gconf settings I noticed that
/apps/evolution/calendar/notify/calendars is blank ([]) even though I
have the calendars checked in Evolution, last_notification_time is -1,
and programs is blank ([]).
