I subsequently did a clean install from CD - I had done a number of 
distribution upgrades over the last three years and many other posts 
suggested that it is is useful to do a clean install sometimes.

Hence there is now no log file in /var/log/dist-upgrade.

Thank you for following through on this one. The clean install solved 
this problem.
