Hi, thanks for your bug report.
I'm marking this as a duplicate of bug # 794060, which has a fix in the next update of software centre (4.0.4).
Unfortunately I've just realised that my fix doesn't quite work so this one is likely to be fixed (properly) in a later version.

Bug description:
  Binary package hint: software-center

  I use most recent version of Ubuntu and have all software up-to-date.

  How to reproduce:
  - open software center, go to an app which is reviewed and click on this review was helpful

  Wat happens:
  - now all reviews are added for a second time to the page: duplicates

  Not a very big deal, but a bug nonetheless.
