Bug 658865 had info about installer (I knew nothing about it).  So I
made couple more tests.  I used a 8 GB USB stick. It had other files
also, including some Release files from Ubuntu and Debian mirrors.  My
guess is that installer program found those, when looking for archive
from CD. It did not return 'CD not found' when archive test failed.
Instead it returned, according to syslog:

Apr 21 11:51:51 ubuntu apt-setup: warning: /usr/lib/ubiquity/apt-
setup/generators/40cdrom returned error code 1; discarding output

That caused installer to crash.  I verified this by deleting those
extra files from stick.  After that installer worked perfectly!

Perhaps 658865 had same situation.

syslog has:

Oct 12 01:27:31 ubuntu apt-setup: Found label 'Ubuntu 10.10 _Maverick Meerkat_ - Release i386 (20101007)'
...
Oct 12 01:29:32 ubuntu apt-setup: warning: /usr/lib/ubiquity/apt-setup/generators/40cdrom returned error code 1; discarding output

Perhaps he did 'cat cd.image > /dev/usbstick' before using usb-creator.


So perhaps installer cannot handle 'discarding output' situation
correctly.

I deleted extra files from my stick, but I believe I can recreate
situation if needed.

  I used usb-creator-gtk and ubuntu-11.04-beta2-desktop-i386.iso to boot
  from USB stick.

  In section 'Preparing to install Ubuntu'  I chose 'Download updates while installing' and 
  'Install this third party software Fluendo mp3 plugin...'

  Installer correctly found other OSes. I chose 'install  alongside them'. I had an empty 
  20 GB partition (6). For some reason installer wanted to use only 10 GB and reserve rest for some unknown Ubuntu http://jkn.iki.fi/ubuntu/install1.jpg. Advanced partition tool did what I wanted: http://jkn.iki.fi/ubuntu/install2.jpg

  A window 
  'apt configuration problem
  An attempt to configure apt to install additional packages from the CD failed.'
  appeared. 'OK' closed it. 

  Next, a window 'Installer crashed' appeared. It had instructions to make this report.
  It did not close with 'close' button.  I had to reboot.  I repeated this with language = English 
  and got exactly same result.  

  Requested log files are rather long to dump here so
