recommend testing your CD drive with other media. In the event that is

Bug description:
I downgraded ubuntu from v11.10 to v11.04 because of the well-known
nvidia driver bug, which prevents dual monitors being used. The
installation fails.
Your reporting system only allows one file to be attached to the report. I have attached the /var/log/syslog, since this indicates the problem:
mkdir: cannot create directory `/target/var/lock': File exists
