The log file does indicate that there are errors with the media but
perhaps it is in the CD / DVD drive?

If you are trying to downgrade from 11.10 to 11.04 this is an
unsupported operation and it will actually fail as documented in bug
891711.  If this is the case what state is your system in?

Bug description:
  I downgraded ubuntu from v11.10 to v11.04 because of the well-known
  nvidia driver bug, which prevents dual monitors being used. The
  installation fails.

  Your reporting system only allows one file to be attached to the report. I have attached the /var/log/syslog, since this indicates the problem:
  mkdir: cannot create directory `/target/var/lock': File exists
