Its likely that something is not configured properly with apt-cacher-ng.
Looking at the source of the package there seems to be a mirrors file
that apt-cacher-ng uses.  Perhaps the mirror you are using isn't in that
list or you are using a PPA for some packages.

Title:
  apt is broken with apt-cacher-ng

Status in “apt-cacher-ng” package in Ubuntu:
  New

Bug description:
  I am using apt-cacher-ng on Debian server for several machines in my
  home network. Two of them were already upgraded to oneiric. They seem
  to not working correctlly with cacher. They see some old package list
  on server. It acts as updates were stopped month ago and noting
  comming out. It is only like that for official repsitory, since ppa's
  are ok. Note cacher were working ok for Oneiric too since some apt
  upgrade.

  Only error message i was able to find in cacher's log is:
  1318329661|E|1116|192.168.1.101|403 Forbidden file type or location

  If this is intended, please mark it as invalid. I faced this for the
  first time, and are afraid of downloading lots of data while upgrading
  other 3 computers.
