This looks like a bug in the installer itself, rather than a problem
with the hard drive. Looking at the log files you've attached, there
doesn't appear to be an errors related to the disk (you'd see read
errors if there was).
Can you describe in more detail the steps you took, and at what point
the installer crashed?
Thanks!

Title:
live Ubuntu CD runs but won't install/run on Hard drive
Status in “ubiquity” package in Ubuntu:
Incomplete
Bug description:
semi-novice user . . . learning by trial and error. Goal was to make
computer more secure/private. Alternating Gparted live CD with Ubuntu
11.10 live CD . . . I gave up experimentation (trial and error) and
tried to install Ubuntu 11.10 onto my entire hard drive as a solo
operating system so I could put the computer to bed (then I could go
to bed). Now the hard drive acts like it is dead. Ubuntu won't live
there anymore. With the internet, to fix problems (like this trouble
report) the Operating Systems & hardware are no longer hobby-material
you can play with at home.
