We did a ver throughout diagnose with apw which determined that the bug
was sitting on user level rather than kernel level, I'm subscribing him
to the bug as well so he can provide any further feedback about it.

Bug description:
  After running a do-release-upgrade -d on a natty server, I'm unable to
  boot the machine properly.

  It looks like it doesn't see the local disks anymore.
