Looks like the problem is in migration-assistant:

  ubiquity crashes installing on md raid1 array

Status in “ubiquity” package in Ubuntu:
  New

Bug description:
  I created /dev/md1 raid1 from /dev/sda1 and /dev/sdf1 for /boot partition and
  /dev/md4 raid1 from /dev/sda4 and /dev/sdf4 for / partition.
  Then I ran ubiquity setting the partition accordingly.
  Ubiquity crashed after copying files to the disks.
