seems that there is a problem with the disk to which you are trying to
install Ubuntu.  I'd recommend performing a back up of your existing
operating system and then investigating the situation.  Measures you
might take include checking cable connections for your disks and using
software tools to investigate the health of your hardware.  In the event
that is is not in fact an error with your hardware please set the bug's

  Installer Crashed

Status in “ubiquity” package in Ubuntu:
  Invalid

Bug description:
  My HDD failed and I'm using an older replacement HDD to get my machine
  back up. I ran the installer once, when it was done I removed the CD
  and re-booted the machine. I interrupted the boot process to change
  the sequence back to the HDD, and let the process complete. When the
  machine came back up it had a "Load Error" message in the top left
  corner of the screen, and that was all. I tried to re-boot the machine
  again, and it came up with the same error.

  I re-started the machine again, interrupted the sequence again so that
  it would boot from the CD-ROM. Once the machine was running on the CD
  I tried to re-install the OS. About half way though (or so) is when I
  got the OS Crashed message.
