I believe this is where things went wrong:

Title:
Upgrade 10.04 to 12.04 Beta 2 failed, grub issue

Status in “grub2” package in Ubuntu:
  Confirmed

Bug description:
  I upgraded a 10.04 to 12.04 Beta 2.

After the compulsory reboot, I ended up stuck on a GRUB shell.

  GRUB didn't get properly updated

  Screen shot is attached.

I know how to pass that problem manually by upgrading grub
configuration by hand using a LiveCD.. But obviously, for your lambda
users it's going to be a massive blocker.
