[Bug 988154] [NEW] I can log on to ubuntu first time of installation then it cashes for the next attempts

I installed Ubuntu several times but i can log on to it for the first time only. then it crashes for the rest of the attempts.
Kernel Panic
init:   plymount main process (353) Killed
Device reported invalid CHS Sector 0
etc.

I cant fix the problem even afer several times

I am running a Windows 7
also
i dont have any problem with it.
