  dist-upgrade from 11.10 to 12.04 through Update Manager stops midway

Status in “update-manager” package in Ubuntu:
  New

Bug description:
  I am using Kubuntu 12.04, and before that I used 9.10, 10.04, 10.10,
  11.04 and 11.10 consequently. Every single time upgrade from one
  distro to the next through update manager failed midway and I am
  forced to upgrade manually.

  The updates download and extract fine, it is the installation phase
  where the process halts. The manager window becomes blank and disk
  activity ceases.

  I have attached the contents of the 'dist-upgrade' folder below.
