The info you posted does not show a /boot partition, was it taken before
you created one?  Did you try reinstalling grub without creating a /boot
parptition?  What exactly does "could not boot any more" mean?  What
*did* happen?

Status in “grub2” package in Ubuntu:
  Incomplete

Bug description:
  1) Installed Ubuntu 11.10, no problem.
  2) upgraded to 12.04. Result: the computer could not boot any more.
  3) created a separate /boot partition at the start of the disk, then reinstalled GRUB in it. Result: the computer is now able to boot.
