but you can ignore that because I am using a USB Pen Drive with Persistence 
File, installed using Unebootin

the failing HD is immaterial to the issue.

Ok.  According to the dmesg output included in your bug report, you have
a failing hard drive:

This may just be a bad cable, or the drive itself could be failing.
Either way, it's not a bug in sysvinit.
