It turned out that this has been caused by RAM fault, removing one of
the modules fixed the problem.

Bug description:
  I'm trying to install 12.04 to a computer with 768mb of RAM. Previously I was able to install the same operating system to the same machine with less ram (probably the kernel was different), but now it fails everytime at the exact same point.
  The same bug exists on 11.10
