You'll need to manually resolve this issue by removing some old kernels
from the system.

While upgrading to Ubuntu 12.04 (LTS), the installation process was
unable to install initramfs-tools 0.99ubuntu13.1. I'm not sure what
that package is or what it does, but it was an error encountered
during the upgrade process, so I thought I should report it.
