In the syslog log file we see the following:

Feb  5 05:24:39 ubuntu ubiquity: dpkg: unrecoverable fatal error, aborting:
Feb  5 05:24:39 ubuntu ubiquity:  files list file for package 'gnome-accessibility-themes' is missing final newline

Given that this package appears on the CD it seems likely that there is
an issue with the disc from which you were installing Ubuntu.  I'd try
creating one again and installing and see if that resolves the issue.
