It looks like you have a major problem with your hard disk.  Open the
disk utility and check its SMART status.

  OS install crashes when copying files

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
  When I try to install ubiquity onto the hard drive from a Live CD or a
  USB, the install crashes when copying files to the hard drive. Knoppix
  installs fine but Ubuntu 12.04.1 LTS does not. The install window
  disappears and the mouse cursor stays spinning in the working mode.
