this bug was in Ubuntu 12.04, 12.10 and I have this "log off only" issue in Ubuntu 13.04 too.
Sometimes it works fine and sometimes system is log off only.

Bug description:
  Binary package hint: sysvinit

  Description:	Ubuntu lucid (development branch)
  Release:	10.04

  When requesting shutting down or restarting the system from the
  graphics interface, the command executed by the system is log off
  instead. Normally, I expect to power off the system when giving the
  shutdown command and to reboot the system when giving the restart
  command. But I see only the login window, no shutdown, no restart.
