Hardy and intrepid have reached end of life.  Have you had this issue
with 12.04 or later?

Bug description:
  Configuration:
  PowerMac G4 MDD Dual 1GHz, 1GB RAM, ATI Radeon 9000 Pro (RV250) w/DVI and ADC

  Installer stalled with message that it couldn't mount the CD. My work-
  around was to burn a 2nd Alternate CD and boot it from an external CD
  drive with the duplicate CD in internal drive.
