So this actually turns out to be an odd interaction between grub and the
"Fast Boot" BIOS option on this laptop. I turned that off and everything
is fine. I installed older versions of grub going all the way back to
precise and the behaviour is the same.
When that option is enabled, grub seems to be unable to blank the screen
or change the video mode. VTs don't work and I can't get any kernel boot
messages, and grub with GRUB_TERMINAL=console draws over top of the BIOS
splash screen without ever changing the video mode.
The help text for Fast Boot just says that it boots only from the
internal hard drive or PXE, skipping the other options such as CD-ROM
and USB. It doesn't say anything about video modes.

Bug description:
This is on an HP EliteBook 8560w with a Quadro 1000M. With nvidia-319,
instead of Plymouth, all I see is purple (same colour as GRUB) on the
top part of my screen and the BIOS splash screen on bottom, until X
starts and lightdm comes up. When I try to switch to a VT, all I see
is a black screen with a purple strip at the top, but input works
since I can reboot with Ctrl-Alt-Del or even log in and run commands
without being able to see what I'm doing.
With nvidia-310 on the same system everything is normal and Plymouth
and VTs work.
