Can you do a 'cryptsetup luksDump' on the device?
Its just not possible that a device gets uncrypted without any password or keyfile.

Bug description:
This was a bug some time ago. Very upsetting when you have your
passwords stored on a USB stick and anyone plugging it in will see the
unencrypted files!
It was fixed, but now appears it is back.
