You have a hardware problem accessing your drive.  It looks like it is
either the controller is bad, or more likely, the sata cable.

Bug description:
  Unknown error caused complete system failure, rebooting did not solve
  teh problem, file-system was locked to read only causing the system to
  become unusuable. Attempted to reinstall Xubuntu but unable to do so
  due to file system being read only which caused the installer to
  crash.
