Are you sure you booted from the usb and didn't use wubi?  Your log
files show that there was a damaged ext2 filesystem mounted on
/dev/loop1.  If you can repeat this, can you open a terminal and run the
following commands and post the output:

df
sudo losetup /dev/loop1

  I have a old windows 7 PC that I fromated and i decieded to install
  ubuntu but when I tried to intsall 12.04.3 LTS it crashed for no
  reason.( I was booting of USB)
