Same for me. USB stick loaded with pendrive tool delivers exactly the
error as described above on my standard PC which usually boots every
sort of linux system without any problem. I have checked the ISO and
tried multiple times. No installation is possible.

Bug description:
  I've downloaded dccff28314d9ae4ed262cfc6f35e5153 md5 hashed
  ubuntu-14.04-desktop-amd64.iso using pendrive linux tool I've made
  installation USB.

  After plugging on USB and choose "install ubuntu"

  This error occurs:

  (initramfs) mount: mounting /dev/loop0 on //filesystem.squashfs failed: input/output error.

  Can not mount /dev/loop0 (cdrom/casperfilesystem.squashfs) on //filesystem.squashfs.
  "

  Using HP EliteBook 6930p as notebook.
