Hi,  I didn't remove the USB stick at any time during install.

I am installing from an image on one USB stick (created using this
http://www.pendrivelinux.com/) to a blank USB stick.  Both sticks are
16GB in size, the target USB has is ext4 12GB / 4GB swap.

Neither we unplugged during the install.  I tried twice and had the same
issue.
