Your installation medium is corrupt.  Please recreate it and/or run the
check disc option from the boot menu.

  12.04 instalation crashes

Status in “ubiquity” package in Ubuntu:
  Invalid

Bug description:
  i cheched the hash number, is ok (iso image). i am certain that the hdd is also good.
  at the end of installation it crashes. error number 5.
  tried to install on the same hdd via different pc, same result
