[Bug 1485251] [NEW] Adding APT Sources from CD fails-reports finish-continue test=crash

Public bug reported:

A window pops up and goes away stating failed to add cd apt sources.
Installer then says Install completed. I chose continue testing to
investigate if it had indeed installed. Upon clicking continue testing
window stays and the debug agent pops up auto. This is on an Inspiron
3520 w/ Win8.1 (preparing for Ten). I loaded the dvd to ram and chose t
install the bootloader to /boot also mounted "Bulk-Storage onto /media
/Bulk-Storage as ext4. These are the only abnormal options I chose,
Which Mint handled fine.
