  Installation fails on LVM drive with no /boot partition outside the
  LVM

Bug description:
  Installed Ubuntu to a broken system using the USB stick that was
  already running with UEFI and LVM.

  The Ubiquity installer should send a warning before you finish
  partitioning that installing without a /boot outside the LVM might
  fail, with the choice to continue or edit partitions.
