As it is a server image, the Bug Squad advice is that debian-installer package may be more appropriate than ubiquity (which is for graphical desktop installs).
If I am wrong, hopefully someone that looks at bugs in debian-installer may have a beter idea where to assign it.

Bug description:
  Hello,

  if one converts the iso image of Ubuntu Server LTS 12.04.1 to the USB
  version with the help of hdiutil, as documented here:

  http://www.ubuntu.com/download/help/create-a-usb-stick-on-mac-osx

  the installer is not able to mount the installation media. It searches
  for the DVD drive, but then errors out, with a "not found" message.

  I switched to a different console, and was able to mount /dev/sda1 as
  iso9660 filesystem. I've also found some errors, visible in the
  attached screenshot.
