Thank you for taking the time to report this bug and helping to make
Ubuntu better. We noticed that you were installing the LTS version of
Ubuntu using an older version of the installation media. The latest
point release of 14.04 is 14.04.3. Please try installing from that
version of the installation media and see if that resolves your issue.
Thanks in advance!

Title:
14.04.2 crashed during first installation
Status in ubiquity package in Ubuntu:
Incomplete
Bug description:
I'm not sure exactly why the crash that occured on this first
installation on an old Thinkpad R61e automatically brought me to a bug
report here. It didn't give me a chance to verify if I made some
error myself or had a dud installation DVD or what, so at this stage
the only bug I feel it's reasonable to report is that the bug
reporting process came up automatically without my intentionally
asking for it.
