Your disk is currently partitioned to boot in bios mode, but you booted
the 16.04 installer in EFI mode. A warning message prompted you that
this could be a problem and recommended performing a bios mode install
instead, but you chose not to. You either need to perform the bios mode
install, or partition the disk using GPT and set up an EFI system
partition to install in EFI mode. This will be done for you if you
choose the "use entire disk" guided install option.

  16.04 installer crashes when trying to install bootloader

  Invalid

Bug description:
  I was booting from 16.04 server edition.

  This OS was made bootable on a 16Gb USB stick by using the make-boot-
  disk tool until 14.04.

  install was:
  * no third party install
  * no download (this option hangs for me)
  * custom partitions:
  ... /
  ... /var
  ... /usr
  ... /home
  ... swap

  all was well until it went to install the bootloader, which failed.

  prior to that it gave a warning about installing - can't remember,
  something to do with UEFI.
