[Bug 1592629] [NEW] New install from live CD failed to install Grub efi

Public bug reported:

Was installing Kubuntu 16.04 from a live CD. The system loaded up til
about the end. Then it failed to install Grub efi and reverted to the
live CD image. The system will not boot.
