Happened to me when upgrading from 15.10 to 16.04 Beta 2. The UPS
battery was fully charged, but during the upgrade the battery icon
showed the battery as fully discharged and the computer was shut down
before the upgrade completed.

Bug description:
  In the midst of upgrading to Ubuntu Desktop 15.10 on 2 different
  computers, each connected to different electrical circuit and a
  different, separate UPS, a Critical Low Battery Warning was displayed,
  followed shortly by a power-off shutdown. In both cases the installed
  kernel was present in GRUB, but would not load. I was unable to start
  any kernel normally in GRUB, but I was able to make a Recovery restart
  with the previous kernel in GRUB and complete the upgrade.
