Your installation medium contains a mix of files from several different
releases.  Before reusing a USB stick for another release, you need to
format it.

   Ubuntu installer failed: The 'grub-efi-amd64-signed' package failed to install into /target/. Without the GRUB boot loader, the installed system will not boot.
