[Bug 1675750] [NEW] Upgrading a VPS from 12.04 LTS to 14.04 LTS with do-dist-upgrade

Public bug reported:

Was on 12.04 LTS
Ran do-release-upgrade as root user (not sudo - but actually as root)
Trying to get to 14.04 LTS, although at no point in the process did it ask me which version of Ubuntu I wanted to upgrade to.
The procedure seemed to go through every stage but at the end a number of fatal errors concerning Apache2.
The files requested are attached.
