This is very likely a duplicate of LP: #1766312/LP: #1765939 which has
been fixed in shim-signed 1.34.5.  It is possible that this also
explains why you didn't see a prompt on reboot: the path which triggers
this bug is if you don't enter the same password twice, and if that
happens two times the package maintainer script will break without
saving this setting.

After installing this new version of the package you will want to run
'sudo update-secureboot-policy --enroll-key' to complete the secureboot
policy change.

Bug description:
  This occurred after I rebooted after an update on April 23 2018. After
  the update, I was prompted to create a password for something
  regarding UEFI secure booting, and the prompt said I would be asked
  for this password when rebooting. When I rebooted, however, I was not
  asked for the password, and had to force a shutdown. I booted normally
  next time, except for receiving this error message. I don't know for
  certain if these events are related.
