The log file /var/log/dist-upgrade/main.log would be useful as that
contains information about the modification of sources.list e.g.:

  deb-src entries in sources.list are not updated from xenial to bionic

Status in debian-installer package in Ubuntu:
  Incomplete

Bug description:
  After an upgrade from 16.04 to 18.04 deb-src entries in
  /etc/apt/sources.list are still set to xenial rather than bionic.

  These lines are commented out by default so it's not an issue until
  the source repo is enabled. For instance a script that automates
  enabling source repositories may just uncomment the line, however in
  this case additional logic would need to be added to also update the
  release name.
