Can you run sudo efibootmgr -vv and post the output? Also dmesg | tail
after that.

Bug description:
Installing Ubuntu 18.04.1 over an existing Ubuntu 16.04 failed because
the boot loader could not be written. The boot drive is an SSD drive,
so hard error seems unlikely.
