Thanks for filing this, sorry it was missed.  Is this a Dell system by chance?
There was this issue related to the built in entries reported a while back?

I's been fixed in current versions that are in more recent versions of Ubuntu.
Would you mind re-checking on a more recent version of Ubuntu?  Preferably if you could test with an Ubuntu 18.04 daily image.  Since you could so easily reproduce it previously you should be able to just create live media and reproduce in live USB mode rather than needing to install.
