I have the same problem right now, I went through multiple how-to's and
none works... stuck on 15.04, and I want to update to anything usable.

  cant upgrade EOL 15.04

Status in ubuntu-release-upgrader package in Ubuntu:
  Triaged

Bug description:
  A user in #ubuntu complained about no beeing able to upgrade his EOL
  15.04 (cloud) server install, so i made a 15.04 server install in vbox
  to test this scenario, and he is right:

  Running "sudo do-release-upgrade" results in: 
  "An upgrade from 'vivid' to 'xenial' is not supported with this tool."

  An EOL Upgrade with changing the sources.list to use the old-releases
  doesnt work since vivid is not moved to the old-releases yet: http
  ://old-releases.ubuntu.com/ubuntu/dists/ .
