Looking at your VarLogDistupgradeAptclonesystemstate.tar.gz file your
/etc/apt/sources.list file is misconfigured:

  1 deb file:///home/vaibhav/repo/ /
  2 deb http://ftp.ca.debian.org/debian unstable main

You don't have any mirrors of the Ubuntu archive in it so it is not
surprising that ubuntu-release-upgrader failed to do anything. You'll
need to sort out your sources.list file before you can use ubuntu-
release-upgrader.
