Public bug reported:

trying to clean install 18.04 LTS bionic on new 2Tb SSD over (formerly)
16.04 LTS xenial (Dell Laptop Inspiron 15 5555). Have installed
successfully with install DVD before (then screwed up config by copying
ALL the config files!), then went back to 16.04.  This played havoc with
all my Eclipse (java) files, so now I'm back to trying to clean install
18.04, and then SLOWLY apt-get all my apps & config files the old-
fashioned way.

This is my fifth time in a row to reinstall.  Each time, it went to the
Dell diagnostic for a disk check.  The install before this, with F2
(setup), there was no "ubuntu" on the bootup choices, and when I
installed on this go-around, I got a display saying the grub-installer
didn't install correctly.

I've enclosed these:
uname -a > uname-a.log
cat /proc/version_signature > version.log
dmesg > dmesg.log
sudo lspci -vvnn > lspci-vvnn.log

but with the last one, I get:
pcilib: sysfs_read_vpd: read failed: Input/output error
